Clavicle fracture with a third fragment supporting the coracoclavicular ligaments – Arthroscopic reduction and plate osteosynthesis

Some displaced clavicle fractures involve a third bony fragment to which the coracoclavicular ligaments remain attached. Arthroscopic assistance allows for controlled reduction of this fragment and preservation of the ligamentous insertions, whilst the fracture is stabilised using an anatomically contoured plate secured with screws.

This technique aims to restore the anatomy of the clavicle, ensure stability of the shoulder girdle and promote optimal bone healing.

The main risks are delayed union or pseudarthrosis, secondary displacement, irritation caused by the implant which may require its removal, shoulder stiffness, infection, as well as rare injuries to the vascular and nerve structures located near the coracoid process and beneath the clavicle.
