Reconstruction of the long extensor of the thumb (LET) using a transfer from the extensor proprius of the index finger (EP2)

A subcutaneous rupture of the long extensor tendon of the thumb (LET) results in an inability to extend the tip of the thumb. Due to tendon retraction, direct repair is rarely possible.

The standard treatment involves transferring the tendon of the extensor digiti indicis (EP2) to the LEP tendon. The index finger retains normal extension thanks to the extensor digitorum communis, whilst the transfer restores active extension of the thumb.

The procedure is carried out on an outpatient basis, under local anaesthesia. Temporary immobilisation is followed by rehabilitation using a protected mobilisation programme. Recovery is gradual over several months and, in the vast majority of cases, results in the restoration of satisfactory thumb function.
